Wmo consultant CMD

Will you become our new colleague?
We are seeking a consultant CMD, an experienced professional with a broad helicopter view and knowledge of the Wmo to complement the team of consultants Wmo in the face of increasing numbers of applications.
Who we are.
Son en Breugel is a lively and green Brabant municipality with over 17,000 inhabitants where it is good to live, work and stay. The location of business park Ekkersrijt within the municipal boundaries, also provides urban activity and matching spatial challenges.
From our new office building in the center of Son en Breugel, we work with 80 colleagues together with residents, businesses and social partners on our local tasks in the areas of spatial development, economy, sustainability, housing, environment, welfare, safety and mobility.

CMD
The CMD Son en Breugel consists of a network of professionals from the zero- and first line who work together integrally to provide customized support to residents of Son en Breugel. These professionals are divided into different areas of expertise such as the front office, Wmo, youth, compulsory education, housing, participation and at various levels. These professionals have in common that they all work in the social domain, can identify and connect broadly and thus ensure a comprehensive approach from prevention to safety.
What will you do?
- Providing information, advice and support to clients in the areas of housing, welfare, income support and Health.
- Provide a signaling function and ensure screening of clients.
- Handle applications within the framework of the social domain in particular Wmo (old and new Wmo).
- Taking care of (medical) indication and the analysis of the demand for care.
- Advise on decisions to be made using laws and regulations, case law and municipal policies.
- Preparation of reports and dispositions.
- Maintain external contacts (including with clients, healthcare providers and consultants) and review bids and plans.
Other tasks
- Occasionally, the counselor may be asked to assist in the Front Office and answer or relay questions from residents or others by phone or email.
- The consultant participates in process-oriented and case consultations.
What do you bring?
You have several years of relevant work experience in the field of WMO. Experience with youth care is an advantage.
You are focused on, together with the chain and network partners, achieving and maintaining quality in service delivery to residents/clients.
You have excellent communication skills.
You advise on the basis of arguments and convey expert and clear advice.
You make decisions based on available facts and alternatives supported by your own experience and expertise.
You are and remain professional and aware of current developments within your field.
We also assume that you have a flexible and customer-oriented attitude, can work in a team-oriented way and are able to achieve set goals.
Additional requirements
In addition to the competencies described above, the following requirements are also placed on you:
- HBO working and thinking level based on education relevant to the position.
- A generalist and systemic view of household, environment and network, in other words, client-centered, system strengthening, demand- and solution-oriented, environment-oriented and outreach work.
- You can switch well internally and externally within a multidisciplinary team.
- You are able to work within a dynamic environment.
- You are proactive and work results- and solution-oriented with an eye for the required process.
- You have an integral view of the client's position and social environment.
- Knowledge of Social Map.
- Digitally proficient given current situation with corona measures.
- Several years of experience within the same type of position.
- Experience with Suite Social Domain and/or similar system.
We offer
An independent and varied job in a developing organization characterized by a business-like character, short communication lines and a good and open working atmosphere. The key position is valued at level 9, maximum € 4208 gross per month based on 36 hours per week (salary level April 1, 2022).
